The City of Oregon is partnering with the Lucas County Health Department to hold a flu vaccination clinic on:
Wednesday, October 7th
9am – 12:30pm
Oregon Fire Station #41 – 1040 S. Wynn Rd. Oregon, OH 43616.
This clinic if open to the public. Please bring your ID and insurance card.
If possible- Please complete the below consent form and bring it with you. This will cut down on wait times however, you can always complete the form while you are there that day.
*If you do not have insurance, they do ask that you pay $21.25 per vaccination however, no one will be turned away due funds.
